They’re Out of Prison. Can They Stay Out of the Hospital?

As aging prisoners are being released from prisons, health care providers are facing a new population of patients needing care. Many of the ex-offenders have not only physical illnesses but also mental health illnesses and addictions that, if left untreated, can land them back in prison. 

Clinics are working with this population to help them transition to regular care, offering services for both the physical and mental illnesses along with mentors, many of whom are ex-offenders themselves, to help the newly released prisoners navigate life and the health care system.
